New World Computing's Music (M) format (the name is a guess) was used in their Might and Magic series, for games 3-5. They contain interwoven MIDI and FM synth commands for complete compatibility between cards with support for FM (such as AdLib and Sound Blaster) and MIDI (such as Roland MT-32 and Wave Blaster). Unfortunately, the games don't let you specify a different device for music and sound effects, so for devices that don't synthesize sound effects very well, like the MT-32, it can be quite disruptive.
