Game Over ~ Name Entry is a song that plays either when you've lost all your lives, or when you make it to the high score entry.
The title comes from the album Galaxy Force. This song is the last section of part 3 of track 2. It's also track 9 on Sega System 16: Complete Soundtrack Vol.2, where is is merely titled "Name Entry".
