The MSX port of The Goonies is different from every other port of the game. The stages are completely remade and quite large, the monsters are different, and there are new items and treasures to collect. While the game is still a platform action game where you run, jump, and climb ladders, there is a major story change. Rather than play Mikey and save the remaining Goonies, you play as Sloth who must rescue all the Goonies. Also, rather than bomb doors open, collected keys are used to unlock the doors which contain Goonies or power ups. The final door opens after the last Goonie is rescued.

Despite the MSX version of the game using a completely different game engine, nearly all of the music and sound effects come directly from the Famicom port, even both versions of Cyndi Lauper's The Goonies 'R' Good Enough. All of the music is rendered on the MSX's PSG. The only new music for this port is the title tune. It is still unknown who arranged the music for the MSX and who composed the title theme.
